I know of two reasons why someone healthy would be unable to get hard:

1. Stress. Masturbation can help deal with mild stress but if you are very stressed it can make you not at all horny and unable to get hard. In that situation if you tried masturbating it probably wouldn't get anywhere,

2. Brewers droop. It is possible to drink so much alcohol that it stops you getting hard. In that case you often still feel horny and if you try masturbating you may get to cum even without getting hard. No much good for sex though.

If your not hard, it's likely your body's way of telling you that your not horny enough to jerkoff. As for me, if I'm feeling horny, I'm either already hard, or am as soon as I start masturbating.
