My younger brother has a stomach nissen (http://en.wikipedia.org/wiki/Nissen_fundoplication)

Well, that should prevent him from puking, right? He threw up earlier today. I don't understand why. Should I be worried? He also has a fever of 103. His lips are occasionally turning blue, but they are pink now. I need to stay up all night with him and make sure he doesn't choke on any vomit, make sure his heart rate is ok, his fever is down some, and that he keeps his oxygen mask on.


Like I said in the title- only answer this if you have experience in the medical area of school.



What should I do if he were to vomit again?
What should I do if his pulse ox beeps and his oxygen mask is still on?
William's nurse told me that if that happens I should just wake my mom up...but, she already took her medicine and she won't wake up unless it's an emergency. In this case- it won't be.

I'm sorry for posting as I'm not a medical professional and neither do I have much medical knowledge, my dadsa pharmacist, does that count? :whoops:

Anyway, if he throws up again or his moniter starts to beep then I would stongly reccomend you dial 911 or whatever the number is over there. I know I'm stating the obviouse and sorry if I sound patronising.

If he throws up I guess just clean it up and make sure he does not choke on it and make sure there is no blood or anything like that.

Sorry I'm not of much help, I know VT has been gifted with paramedics and other medical proffesionals do hopefully they will be along to help you out soon.

To help to prevent him chocking on his vomit ( if that is a danger) lay him in the recovery position or on his side and wake him up.

Ok Medic Here,

Its a really difficult situation - The Cyanosis (Bluish Tone to his lips) is the lack of Oxygen reaching his vital tissues. I would recommended seeking Medical Help ASAP. If his SP02 (Oxy) Beeps again whilst he has his 02 (Oxygen On) - Call an Ambulance, or contact the Nurse asap. If he vomits again, try and keep the airway clear if possible - this can be rolling him to to side and making sure the Fluids/Objects are clear (Drain). This is really vitally Important.

If you have any doubts or he turns blue again, fever returns or vomits - Seek medical aid. Any questions - post them here and I will return to help when I can.

I totally agree with what Josh has to say, also if at anytime you feel that you do not know what to do you NEED to call 911 or the equivlant, also wake up your mother. Seeing that you have do not have any medical experience (please do not take this as me taking a stabb at you) you need to wake her up.
The best "medicine" that you can provide is immediate care(calling 911, your mother) the faster you can act the better it will be. Also a good thing to do is write done things as they happen, one of the first things we, as EMTs are to do is get a history and by you writing things done saves us time so we can provide the immediate care that we need too. If he turns cyanotic(blue) it is due to an oxygen deficency or vomits and starts chocking he needs to be treated immediatly, that is why we have EMS.
Also having a list of his medications that he is taking on hand can help during an emergency.
If his Pulse Ox drops below 92 call 911 and the nurse. I hope this helped, PM if you have anymore questions.

thanks for your help guys. We don't really...call the ambulance here for my brother. To be honest, we have everything here that a hospital has. The only difference is that the nurse wouldn't be there from 10pm-6am. His nurse just didn't explain what I should do. Last night , though, my mom ended up waking up to go to the bathroom. I walked in to check on him and she was just starting at him..I was so scared, I thought something was wrong. But, in the end, she was just watching him sleep (awww) and she had me bring him in the living room so I could watch him better.

Sorry for responding, i'm no medical expert, however if your mom wont wake up to help then the best thing to do would be to call 911 if there are any complications. You wont get in any kind of trouble, and it could save his life if it is serious. However the article said it has less than a 1% mortality rate, so he should be fine, but if not, it's always best to contact someone for help, and if you call 911 your sure to have assistance fast. Also, emergency or not, your mom should wake up if there are complications, her medication or not. It's her son, and she needs to be there for him. But it's best not to worry yourself sick, because that will only make it worse. Just keep an eye on him and if you notice anything wrong, try and wake your mom. If it seems something bad, or could become bad, don't be afraid to dial 911, it's your brother, after all.

I also agree with Levi (Seems to happen alot :P) You may have medical equipment there - but if the nurse isn't there, there is no one there to operate the equipment. I can understand where you are coming from - but if has a low SP02, whilst wearing the O2 mask, he needs help. Choking is also a no brainier - Just call the EMS. They will not go off at you for calling, but they will not be happy if you wait for the condition to worsen.

Like Levi said, it is good to have medications and medical history and all details printed up and stapled together hand to the Paramedics if they need to come. If you do call them put all medications in a bag, and record his SP02 every couple of minutes.

I would also be talking to the Nurse to ask, what to do if it happens again - get her to write it down with a number she can be contacted at.

I will give you a basic fundermental in an emergency you use ABC, a is for Airway, B is for breathing and C for circulation, Blue lips means his circulation is obstructed, his sleeping position needs to be laid flat hands to side etc, this is open circulation, this gives lungs max capacity so breating and airway have an improved ability so less likely to choke, if you tilt the head so at 90 degree to body and the head is level with the rest of body (no incline) vomit will drain through the mouth with excess not beturning sown the wind pipe, the oxygen mask is the sisk as some do have a fluid drain but what we do is check every now and again and clean excess

Isn't it considered child abuse if a parent ignores a child who is dangerously sick? I think you might want to tell another adult about this. I am also proud of you being a big, responsible sister.
