User name or Real name? Ehhh... mmmm... ahhh.... I guess username :P
Anything else?: Brushes would be cool. Like the little skull in yours, except something else (I don't know what you have

Font (for your name NO QUOTES OR LYRICS due to it being 120x120): Oooh. Umm. Jellyka - le Grand Saut (http://www.dafont.com/jellyka-le-grand-saut-textual.font), I guess, to match my sig. Not the font that says "textual" though, the regular one.
User name or Real name? Ehmm. I'm not sure. :P I'm leaning toward my real name because it looks better in the font. :P
Anything else?: Not too over the top, but slightly gothic? I don't know, play with it a bit, see what you come up with. :)
