Its generally cleaner inside than you would imagine unless its a "bad tummy" day, but like Chris said before filming porn stars will always have douched and showered. If you're worried then wear a condom.

Hardly ever happens for us, you just make sure you aren't close to a uh 'bathroom break'. Its gross if it happens but if you start out with a condom you can deal with the problem and then take it off if you want (assuming you have thought about stds). Not really an issue if you keep tissues and baby wipes handy.
