I was just wondering on some tips, what to put, what I shouldn't put, etc.

Most of all just write how you feel.

Try and explain yourself to the best of your ability.

Is there anything significant that is bothering/upseting you the most?

For filling down the referral form for the counseling just make sure what ever you right down is truth of the best of your knowledge and that point would be considered while counseling.

There really shouldn't be anything you're not going to put down. It's tempting to omit things, but really you have to be truthful with counselors, as they'll either find out eventually or they won't be able to help without all the info.

I don't know exactly what's on your form; forms are going to be different in different places. But just be truthful. It's worth it in the long run.
