Did you just pierce it?
If so, did you do it?

Was it recently pulled on, or tugged on?
Have you been playing with it, or moving it around alot?
Have you eaten anything that could've irritated the area?
Any chance it's infected?

Yup, you might want to talk to a doctor. Idk if I would take it out, the hole might close up if you take it out. If it is infected i think it would have puss. Also, I think it would get red if it got infected. I say leave in the piercing, ice your lip, and give it two days. If it is still swollen, gets puss, or turns red, take it out, and goto a doctor as soon as possible, no I don't mean the emergency room unless your lip looks like your about to die.
I hope I helped. :D
