Basically, it is just a person who identifies themselves as a gender opposite of what they are. They may also undergo a complete sex change (which would result in organs attempted to be converted).

The google definition is: A person who has gone through a surgical procedure to become a member of the opposite sex. In a male, the penis, testicles, and scrotum are cut off and a vagina is fashioned into the body. The person can not bear children. They are given hormones to create female tendencies.

Transsexual means that the person is born one sex but feels they are the other. Many/most at some point in their lives will undergo hormone therapy and/or surgery to correct their sex.
Contrary to popular assumption, there are people born female who feel male as well as those born male who feel female.
