In a straight marriage it is a common occurance for the woman to change their surname to the man's surname.
In a gay marriage, who would change their surname? Or would they just keep them the same?

Well, that's not a problem in Spain, because when a girl gets married she keeps her name.

It is becoming more common for women to keep their surname or for them to take a doublebarrelled surname upon marrying.
But with regards to same-sex marriages, I imagine it would be down to individual couples (much like with opposite-sex marriages).

I believe they either get a hyphen or they have the significant others last name. Just like straight couples.
