The hymen is only broken about 1/3 of the time. The only time this happens is if the guy is too large (well above average) or if the girl isn't sufficiently turned on and lubricated. So, generally, unless the guy does something wrong (which many do at first), the hymen shouldn't tear at all.

For girls the first time, it very well may hurt, though, just because their muscles are being stretched beyond what they normally are. However, with proper foreplay and lubrication, the pain should be minimal.

Vaginal sex hurts when;
- the hymen is torn (this also bleeds, but many girls nowadays break their own without realised by playing sports or horse riding etc)
- the girl is not turned on enough so her body hasn't produced enough natural lubricant
- the girl is scared/nervous which as well as making her "dry" (see point above) will cause her to tense which can lead to slight internal bruising of the smooth muscle lining of the vagina

In anal sex i'd imagine it hurts a great deal for the recipient, unless the guy doing the entering has a really really small penis.
