It could be a kidney stone. How long has it been hurting you and how often?

Kidney pain is going to be felt in your mid-lower back, closer to the center than to your sides. If it's more of a stomach thing than a back thing, it's probably not your kidneys.

It's hard to tell what it is, especially if this is a long-standing thing rather than sudden. With a sudden thing, you can look at what things you've changed recently, but I don't know how to diagnose general chronic pain.

I'd see your doctor, to see if there's a reason for it or anything you can do about this.

Well, appendicitis wouldn't stay a problem for so long. Usually the pain will be fairly sudden, developing over a few hours or as much as a day or two, may even be accompanied by vomiting or bowel distress, and will typically lead to the appendix rupturing if it's not treated in time. Appendicitis is often an emergency surgery kind of thing in order to avoid just such a complication--a ruptured appendix will not only cause a little internal bleeding but can cause potentially fatal infection in the abdomen. It can't be left alone for years without rupturing and killing you.
