You dont buy underwear by penis size. When it says "small", "medium" or "large" on the pack that's not what its talking about. So you dont buy new underwear because you penis or testicles have grown. Its all sold either by waist measurement or by age range. At the end of the day you buy whats right for your waist size otherwise they're either too tight or would fall down.
