You may be studying however if your just reading them you may not be taking any of it in, you have to find a method that works for you, like reading things outloud, writing facts/figures out over and over again.
Studying is hard work and your the only person that knows if your working 100%. Try to keep motivated by thinking if you study hard and do well you've got something to look forward to, one reward could be not having to study!

It is clear that your current methods of studying are not working, but don't panic, that just means you have to change them for the better. I would recommend that you read this article it may help you out:

Also you may want to speak to your teachers, they will be able to help you out to.

Studying is not about the quantity or duration you study but rather the quality. One person can study for 10 hours and another for 2 hours and both can get the same grade.
You have identified one major problem: depression about your work. Studies have shown even if you tell someone that they'll screw up, they're more likely to screw up. If you are the person who keeps telling yourself this intentionally or unintentionally then the result is the same or possibly even worse.
You may be studying incorrectly or you may not fully understand the material even before you start studying. I cant tell you a best way to study without knowing what the subject is and my ways may not work for you. On the other hand, your studying method may work for you but the stress during the test and your academic history may affect your performance and you get a low mark.
Essentially, what I see is you don't understand the material properly, you write a test or quiz and get a bad mark, get depressed over it, your depression affects your studying or your test-taking, and the cycle repeats.
When you are going to write your test, try to be calm. Remind yourself that even if you cock it up, it's only a test and it's only one course that you can re-do later. Think positively, review your notes beforehand, ask your friends/classmates for help, etc... .
But as I said above, without knowing how you study, how you take the test and what the courses are, then I probably won't be able to give a whole lot of help.
