Totally agree with everything. I think that this is happening because you don't have the ability to actually cum yet. What is happening is that after climax you are relaxing so much that you end up peeing so consider peeing b4 jacking. You might not cum still though for a while.

This only happened to me once, but I guess I was putting too much pressure on my muscles. Which is why they say to relax when you masturbate. But in your case, since you're only 13, you're gonna have to wait a bit.

It sounds to me like others have already mentioned that you're putting too much pressure on your muscles, and that you should try urinating before attempting to masturbate. If you're putting pressure on while masturbating it is likely that after your climax the pressure you've used has been forceful on your penis and has affected the bladder, thus urine is coming out.

To prevent this try urinating before masturbation and also try to reflax and use less force and pressure in your "grip".
