All above posters are correct, but I think I should add "all the way back" can mean different things. It's not really supposed to go further back than the base of the head of your penis. But you shouldn't worry if it doesn't go even that far back. Personally, my forskin didn't go 'all the way' back until recently (and I'm 18).

A good way to make it less tight is when your as flaccid as you can get (when your penis is only like an inch) pull it back halfway-ish and as you start to get a boner pull back more until it goes over. If you get hard it'll hurt a bit but it helps make it less tight. If you have trouble making it go back up keep pulling it down until a white bit comes out. It's kinda like your foreskin has turned inside out. then slide the white bit onto the head of the penis and it should ho back to normal, then you can slide it back up to the un-retracted position. Only do that if you can't make the foreskin go back up, though.

no. Foreskin is naturally pulled back during arousal. It doesn't need to be pulled back for urination, but it is an option. However, it is HIGHLY recommended that you pull back the foreskin during your shower and wash it a little every day. This helps prevent some diseases.
