I'm 16 and I'm uncut

My foreskin pulls back all the way when flaccid, but at the base of the head (directly below the "urethra") it's attached.

Is this normal? Is it supposed to be attached to the head at all?

And when erect I can pull it back almost all the way, but it starts to hurt right as I get the whole head "uncovered". Is that normal?

Yes, this all is normal.

The attachment you're referring to is the frenulum, it attaches the underside of the 'skin to the penis, extending up to the urethral opening. OK.

The other issue is the dreaded and common 'tightness'. Just pull it back gently and consistently in the shower and when you urinate, and with time, it will stretch effortlessly over the head. Just don't force it and be patient.

The next time you're at the doc, let him have a look-see.

i cant pull my foreskin back, i think i have too much of it, is this normal ? i can still ejaculate though

OP: This is your frenulum. It's supposed to be there.

Tight foreskin is a bugger. Follow Sam's advice. Should be easy for you, because your case isnt very bad...
