I think you're asking if this fantasy (and the enjoyment you get from it) make you gay, does it mean you're gay. And the answer is "No", it doesn't necesarily mean that. Your sexuality does not come from what turns you on.

You might want to think about what it is about this one particular guy and this one particular act that is arousing for you, it might help you understand the fantasy more.
