It hurts because the anus is not meant to let things in, it's meant to let things out. So until it stretches, it hurts.

I don't think it needs to hurt unless the "top" guy is a bit rough and insensitive. You need to take it slow and have lots of lube. It may feel a bit uncomfortable until you get used to it.
