It's a piece of skin that covers your penis. When some children are born, their parents have them circumsized, and the tip of the penis will look like a mushroom, kind of, at all times. However, if not circumsized, you will only see the mushroom shape when erect. When you get an erection, if you're not cut, the foreskin will slide back. Basically, this is a really crude way to put this, it looks like the end of a pig in a blanket (a food that is a cocktail weenie wrapped in dough).
