Does smoking slow down penis growth and is it possible to be in stage 3 of growth at age 18

It might but I would worry about cancer and other diseases instead.

Yes, it does affect growth. Even if you are 18 (since you are still in puberty at that point).

It is also possible to be in stage 3 at 18.

it will continue to grow from where your length is right now, but it won;t make up for the lost growth.
