He asks you to drop your pants and he asks you to turn your head and cough while he feels your testicles. It is some way to tell if you have a hernia, I think.

Its basically like having a normal physical. They take measurements and then feel all around your body for stuff.

Ive never had a sports physical, but basiclly they make sure all your reactions and muscles work properly. They tell you to turn your head and cough while they touch your testicles. They jump up slightly and this makes sure that theres nothing wrong with them.

Whats a hernia?
a hernia is a rupture in smooth*muscle tissue through which a bodily*structure protrudes. Basically when some structure got knocked out of place and cut through some tissues. It can be very harmful and you should go to the doctor if you have a hernia.
I'm not sure if my doctor gave me a proper sports physical or not, I thought it was just a regular physical :-s anyway it was just like a regular physical. Blood pressure, heart rate, checking penis/testicles, testing eyesight.
It's nothing to be embarrassed about, it's the doctors job and you'll be able to be cleared for the sport you want to be in after you get it done! :D

when i was 11, i went to the doctor, and he seemed surprised that i had a lot of pubic hair. that wasn't embarasing. more recently, when i switched doctors, my new one, after i pulled down my pants, i had anerection. the doctor just hesitated for a sec.

I noticed that in the poll you have put two questions asking if anyone has failed a sports physical. You do not have to worry, because there is no way to fail one, it is only to make sure that you are healthy.

Well, if it it's for sports (most school sports teams require you to give them a copy of your "clean bill of health" from a recent physical), it's possible to have some health condition that would cause them to not allow you on the team, or to not allow you to compete (it'd have to be a fairly serious problem, though). But you're right Mzor, it's impossible to "fail" the exam, you're not graded or anything.
If you're nervous about the exam when you go in, tell the doctor, and ask about it. Just be like, "This is my first time having a physical, and I'm a little nervous. Can you explain what you're going to be doing?" The doctor will be careful to not surprise you with anything, and you'll be able to relax a bit.

Ah, sorry, I was thinking of a normal physical for some reason.
And taking Jessi's advice would be good, that's always what I do when I do any new thing with a doctor of any sort, usually not because I'm nervous, more out of curiosity. But now at least you can go in having a rough idea of what it's all about.

For my physical he stuck his finger up my butt while I coughed. He's never felt my balls.:)
