well for people who haven't or are not used to sleeping nude, it is just the feeling of the sheets and stuff on the groin area. I guess for some there may be the kinky factor too, but for many just the feeling they are not used too causes them to get hard. it is like when some people change form boxers to boxer briefs and get boners, it is often because they are not used to the way they feel and have to get used to them. it is not necessarily a fetish
