Mine is completely exposed when hard, the harder it gets, the more it pulls itself back. If your having trouble then speak to your doctor, there are creams they can give you to ease with pulling it back

I would say yes, but it is possible to have sex with it not exposed, just a little uncomfortable I imagine.

I have very thick foreskin so the head is not exposed while erect.
