Smart casual, can't go wrong with smart casual.

Whatever the hell you want. Technically, you can wear anything on a date, if you know the person that you're dating well enough and vice versa, but I would recommend, for a first date going in somewhat formal. (white oxford, khaki, or dark slacks, dress shoes, maybe a tie) to lay down an excellent first impression, and over the number of dates, slowly become less formal, although I would not recommend going in anything stained, ripped, tattered...etc, unless said date specifically calls for that style of dress.
Of course, I've never been on a date before, so this is coming from my instincts.

Yeah, I'll agree with this one. You definitely don't want to over dress. It would look silly if you're there in a tie and a suit and the other person is just casual.

Something nice and not too wild or crazy. Just probably a nice polo shirt button shirt and some nice jeans would be fine.

it depends on where you're going on said date, brah. what's the atmosphere? if you're just going to McDonalds or some other no mucho dinero joint, shorts and a T-shirt will work fine. if you're going to someplace fancier then go more formal like a long sleeved shirt and a good pair of pants. if you under or over-dress for something it ends up looking kinda goofy.

A bright pink spandex bodysuit. You can never go wrong with a bright pink spandex body suit.
 Seriously though if you want actual advice you need to be more specific.

What type of date are you going on? That would decide what you should wear.

Depends where your going on your date. Are you going for a meal? Then I'd suggest a smart casual shirt an jeans. Or are you doing something a little more relaxed such as the cinema etc. I'f so then a nice t-shirt and jeans

Depends on what kinda persn she is. Here in the country, any outfit named above would mean she rolls around on the floor laughing, wondering where the rips are.
