Mental health survey at doctors??

I just got back from my doctors appointment and I'm kind of freaking out. I had to take a survey for mental health and I tried to answer honestly but it was hard too. Then it asked if I have ever tried I commit suicid and I marked yes. And the way the nurse looked at me when she took it, kind of sad and shocked :( god I hate this, my heart is racing. :(

Aww, sorry you had a crap time, butr that survey will no doubt help you in the future, it's not good to hide your past. Just keep it calm, OK? Everything is going to be OK. That survey was designed to help, not hinder

hey Yas, just remember no matter what you have friends. Im not much but by god i will sure as hell stand by your side no matter what. whether friends are close or far, were here. ive lost my best friend over the summer, and went through alot, you stood by me, same as my baby, my true love. so i have your back come hell or high water. just message me. any time, if your feeling down give me a ring. i havent many friends, but i care deeply for those i do have. just know this. talk later.
