Usually during arousal the penis swells, and the testes increase in size from increased blood flow. Orgasm and ejaculation will lead to normalisation of vein and artery size, and return to a relaxed state.

Blue balls is when there is a congestion of blood flow in the testicles. This often occurs if ejaculation does not occur, such as from interrupted intercourse of masturbation. It basically causes anything from very little, to a heavy and dull sensation like an ache downstairs.

Supposedly, the congestion of blood flow can lead to a slight bluish discolouration - but I've never seen this. Perhaps the blue also refers to the sombre mood one can get from the discomfort? ;)

In any case, you're lucky if you've never experienced this! Not something you'd want to happen to you! Haha!
