I HAVE CONTACTED MY GP TO HAVE A PRIVATE PHONE CALL TOMORROW ABOUT THIS PROBLEM, BUT I DON'T EXACTLY KNOW HOW TO TELL THEM ON THE PHONE ABOUT THIS PROBLEM.
Do I say, 'After masturbation I realised that there was a mixture of semen and blood'? I really don't know what to say and I am really nervous, please give me some advice on what to say, of course I am going to keep to the facts but I don't want to sound rude or vulgar on the phone.

Say u ejaculated bloody semen. It might b easier to say than masturbated. They're doctors. U couldn't say anything to them they haven't heard before.

I think you should just be honest about the problem because it is not like the doctor is going to make fun of you or anything. After all, everyone knows that teen guys jack off, and the doctor is not going to clueless. Just tell him/her exactly what's going on, and I guarantee that nothing embarrassing or bad will happen. You're dealing with medical folks, and they are not there to put you down or make you feel badly.

This shouldn't be something to overly stress about. They're doctors, they've seen and heard just about everything. They'll understand what you're getting at.

Firstly, don't panic.
I had the same problem last year. I abstained from MB for lent, and then after 40 days MBed and there was blood in my semen. I went to the doctors and explained that I saw blood in my semen. He asked me how I knew, and if it wasn't blood from a women, etc / act of sex. I explained that I'm a virgin and saw the blood via MB. First, he did a test of my urine and confirmed there was no blood in my urine (something they should be able to do right on the same visit, so drink some beverage before you go in). He'll also do some checks on your package, feeling the balls and pensis for any issues. He gave my some pills to cure infections, in case it was an infection. He also sent me to a lab to do a PSA and further urine tests. A few weeks after, when the tests came in, everything was good.
He explained that there are a lot of vessels involved, and that most cases, especially for teens, is that one of the vessels burst, which is very common. He sent me for all the tests just to confirm.
Something you should check, as he suggested, was to check for the color of the blood. The first time I saw it, it was bright red. When I MBed the next day to confirm, it was more rusty in color. It was more consistent with his opinion it was a burst vessel as it was not still bleeding, but blood remains from when it burst.
Nothing to be worried about. Just be thorough and get it checked out. It wasn't awkward talking with the doctor. Just awkward that he assumed I had sex and noticed it. I think he assumed I was lying, to hide that I was having sex. The pills were to cover that case if I was lying and had contracted an infection or STD.
BTW, he never had me ejaculate in a cup or anything. He was contemplating including a semen test in the lab request, but said the blood work test should be sufficient.

UTI usually means blood in URINE, however, seeing as two things come out of one pipe, I guess it could be possible.
If you're not a virgin, as has been mentioned, it could be an STD.
It could also be what Talkradiogeek described.
Honestly, just say what feels most comfortable to you. Your doctor is not going to judge you for masturbating, and nothing you say will shock him.

If you abstained this year, it could be a factor. Not masturbating has the semen not going anywhere and healthwise things are slightly more likely to go a bit wrong, like a burst blood vessel as you said it could be.

Don't bring up the uncomfortable part. Just tell them straight up what you observed. Say you ejaculated blood in your semen. They are doctors. it's nothing new and even if it was, it is their job to help you.
