I'd never heard of it happening just because a long period had elapsed since you last ejaculated, rather it seems to happen when you spend a long time aroused without finally cumming. The kind of thing that might happen, for example, if you go on a date, get touchy feely short of anything that gets you off, and then go home. In that case the answer is to jack off when you get home.

From the Wikipedia article it doesn't seem to be that well understood but certainly it isn't anything to do with pressure from too many sperm stored up waiting to get out. You correctly note that sperm are a small proportion of semen and furthermore sperm are produced continuously - they don't suddenly get produced when you're aroused in a way that would explain blue balls in the typical long arousal I described above.
