never.

i haven't seen many penises in person, but i don't see any situation where a guy respectfully showed me his penis (in other words a guy shows me his penis because i want to see it) where i would say anything hurtful about the size. i'm just not the kind of person who would hurt someone who trusted me with something so intimate.

I think the reason we ask about penis size is mainly to anticipate what a girl's reaction will be when she sees it rather than someone actually having said something offensive. It's a bit to just confirm that we won't disapppoint anyone if we ever were to get serious about sex and stuff like that.
