How do you even wear this?

I'd like to start by saying: I did not buy this. I saw it though and found it kinda interesting, but also extremely awkward. How does one even wear this without feeling super uncomfortable?! People'd be staring at your nips! I assume it would look weird with a shirt underneath it, but I'm curious as to other peoples' opinions.
