Physique (males - broadening of shoulders, females - broadening of hips and breast enlargement)

Growth spurt

Facial hair (if any)

Acne (if any. Please indicate whether breakout or just an occasional pimple)

Genitals (penis growth for males, menstruation for females)

Sexual Orientation awareness (when did you identify yourself as gay/straight/bi, etc.)



Take note that what's being asked is the FIRST TIME you noticed each development. So for example, for "voice", take into consideration when you first noticed your voice changing.
