Yes, you clean everything, first an enema (injection of water or one bought from a store since enema is a medical procedure done at home) next, in the shower or bath, soap, water, rag, everything.

Then once you are sure you are cleaned up everywhere and especially down there and in your pubic area then you decide if you want to do it or not, use lube because you need to and because that area is tight and for safety.
