How to discuss "us"

Me and my ex from 4th grade are talking again. We like each other and i want to go back out with him. But how do i bring up the "us" topic without sounding creepy or annoying?

I have been in your situation. I don't really know what happened but I just cut the chase and went straight to the point. No twist. I mean I was like 'How long are we not talking?' or simply 'what happened actually, between us?' something like that. You could slip in some jokes if you don't want it to be creepy because he's a guy and it's fine to be 'rude'. I mean friendly and joking kinda rude like ' Hey stranger. Ya know, I like the way we were until you go all ego like that, hahaha' kinda thing. Just suggesting. Be yourself, just talk. If you are a girl, start with 'I miss us' kinda stuff. :)
