The outfit (top+bottom) are sewn together so it is one piece. Like a dress, only with legs.

Whenever I've explained the concept to Americans I get laughed at so hard I swear they actually piss themselves. Yet here in the UK they're so common practically every kid has one, they wear them on non-uniform days and everything. So yeah, the 'trend' will probably make it round there eventually.
