As you get older in the teen years your balls will hang lower more often, like when you're in the shower and they hang really low. It's like they're warmer more often so they hang lower more often. And your voice deepens, that's what that saying means.

Its really just when you are going through puberty. And that includes your voice deepening.

As my parents always told me, its when a young man is starting to enter puberty, and they actually start growing. "Before growing, they are roughly the size of a pea, or perhaps peanut. When a boy starts puberty, they start to grow, and this makes them more noticeable. The bigger they are, the more noticeable they are, hence it looks like "they have dropped." But there are medical conditions called "un-descending testicle" where they get stuck up inside and need to be pulled into place." Can you tie them in a knot? Can you tie them in a bow? If the answer to either is yes then your balls hang low (and probably swing to and fro), and therefore must have dropped.
