Help me figure out name of book title

There's this book I've read years ago and I still remember the plot (sorta) but not the name. It's more for younger kids though, but I don't care about that fact. I just want help figuring out the name of the book (it's the first book in a series).

I'll try to sum up the plot as best I can.

The book is about a boy, who has a serious heart condition. He somehow gets lost, stumbles across the Great Divide (or whatever it was called, can't remember), and appears in another world, where magic and magical creatures exist -- like phoenixes, griffins, and unicorns -- and science is believed to be a myth and creatures like elephants considered to be a mythical animal.


That's all I can think of right now. Can anyone please help me figure out the name of the book? Thanks.

It's called "The Divide" by Elizabeth Kay. It's the first book in a trilogy.

Yeah I think it is. It's pretty interesting. It's not something most older teens would read ...but I don't really care that it's more for younger kids.
