I've seen a lot of threads about seeing your friend's penis and showing, but always on purpose (that is, the reason being to show and tell), but never accidntally. I'll tell you what I mean:

I was at a sleepover with two other friends. We're about to go to sleep and my friend pulls down his pants (he sleeps in boxer briefs and t shirt), and there's his little man peeking out of the flap to say hello (soft, by the way). My friend points it out to him, and he slides his penis back into his underwear. The next morning we wake up, and he's lying down on the floor, half asleep with a giant erection. He wakes up, and I point it out. He sits up and covers it with a pillow and says, all embarrassed, "it's the morning!"

Anyway, I was wondering if that kind of thing has happened to others

Yea only a few times though. One time my buddy took his out because there's a unspoken joke/rule in golf where if you don't hit it past the women's tees you have to play the rest of the hole with your dick out.
