First, you would need to make sure that his weight and his vomiting are correlated. Alot of guys are skinny, that doesn't mean they're bulimic. And, throwing up (once?) doesn't necessarily mean it, either. Cafeteria food can do that to you, ya know?
If you observe this for a little while and it continues, then the only way to help him is to talk with him about it. Do this gently, ask him if he's ok, tell him you've been seeing this pattern. Tell him you'd like to help, and you want to go to the school guidance counsellor with him. If he refuses, or gets real defensive, then go yourself. If all this feels waaaay to intimidating, then AFTER you're really sure he's inducing vomiting, go to the school guidance counsellor yourself.
Doing the right thing isn't always easy.

Agreed, you should be sure before you really say anything. If you observe this behaviour for a while, then maybe you should tell a teacher you know would understand and keep this confidential, who could maybe help you get your friend to go and see your school guidance counsellor who could perhaps refer him to someone who specialises in the area of eating disorders, so he could get some help and try to overcome this.

How about you make sure his not sick.. Some illnesses can make people very skinny and you can't be sure he did make himself vomit.. Ask his parents if their the kind of people to ask.... Or just ask himself is he sick or just find out someway before you ask something like that from him or try to help him with.. As he might be very offended if it would come out to be true.. At first he'll be problably offended anyway is it true or not but.. I mean if it wouldn't be true he'd be possibly very offended for a long time not for a short time
