Yes, it kinda doesn't show any respect for the other person who is in this relationship and puts the person you asked out in an awkward and uncomfortable position.

Generally I would. I mean generally you have to know them well to ask them out so surely you'd know if they were dating.

So yes it is advisable that you at least find out if the other person is dating to avoid bad situations.

Well, I have done this before. The girl was dating this really mean dude who she didn't deserve, and actually she broke up with him because I told her that. However, she didn't end up dating me :/

If you didn't know they were dating someone then there is nothing to be ashamed of. But if you did thats very disrespectful to that persons love interest/bf/gf. If you did make a mistake, just make sure that person knows.
