Well, actually... Legally (in Canada as far as I know, and most likely the USA) they can't tell your parents you harm yourself. If anything they can't even say anything to you about it.

If you are going to get it alone (go into the room without your parents, don't worry about it.

Most likely a nurse is the one who will give you the shot. If she's anything like the nurse I had when I got a vaccine, she will ask you which are you want it in. If both arms have the scars and cuts on them--which I assume they do--she may or may not notice. If she sees them, she will certainly wonder what happened to you there. You might try to pass it off as an accident, or as an injury from working sometime. As far as the United States goes, I think that a family doctor/nurse might be obligated to tell your parents--I'm not entirely sure. In the end, you can do the best you can... I wish you luck man.
