I've always been confused on this.. How does one get banned? And is the icon thingie permanent if one were to be banned?
Also, what are the restrictions on someone when they get banned? Are they still allowed to post, or do they have to wait a certain amount of time before they can start posting again.. Or something like that?

Break the rules enough.
Here's the food chain:
Break the rules ---> Infraction ----> 3 infractions/75 (?) points = frozen (temp ban) ----> anymore = banned
and there you have it, your lovely food chain. You can also get banned if you're overage (joined after 20), um spam bots get banned, and underage members (joining before 13) if caught get frozen for a year.
Icons are permanent for a ban, the frozen icon (light blue, icy) lasts only during the ban, and depending on the freeze, you might not see the blue icon. I believe the blue icon happens only for the infraction buildup/underage. Other times you'll see 'frozen' as someones usertitle, but their normal post picture, which means they were generally a bot.
Banned means banned, you're gone. You can always appeal but it's generally not worth it. I believe when you're banned you can still log into the site, you just can't post or do anything, pm, etc. pretty boring. Freezes work the same except since they're temp (generally 7 days for the infraction buildup, and 3 days for others, and a year for underage.) You'll occasionally see members request to be frozen or banned, at their request in which they will be unbanned.

On to the other parts.
The icon is permanent yes. You can't post, you can't view your profile ect. All you see is a page that tells you you're banned, why you've been banned (assuming there's a reason) when your ban will be lifted, if ever. You can appeal if you think you've been banned unfairly. Other than that, you no longer view the forums, unless you're on guest mode - ie not logged in.

What if you get an infraction that isn't worth any points? And can you please explain expiration of these infractions while we're in this thread?

Infractions without any points are known as official warnings. They come before an infraction. As far as I know they still last as long as a regular infraction.

When I had a warning, I think it lasted around 2 weeks.

If someone gets an official warning (infraction with zero points usually given before an infraction) then that can (I think) count towards the person being frozen when they reach 3 infraction/official warnings they will be frozen for 7 days. However if someone gets two infraction that equal up to 75+ then they will be frozen until one expires. Expiration dates depend on the severity of the infraction. Official warnings expire at the same time as infraction and they all depend on certain other circumstances.

Yep. However sometimes depending on the severity of the case we miss the official warning and go to an infraction.

One last thing to note would be if you had an infraction that was still active and then you given another infraction for the same thing then the expiration time on the second infraction would be longer (I think it doubles).
Also if you get a couple of the higher infractions and they are still active and the total reaches above 100 points, then the person is banned.
