How to ask her out?

I was just wondering if some people could share some success stories with me about people that have asked out and are/were in a good relationship with (not really shy around each other but really comfortable, kissing etc.) - as I need some tips about the normala and best ways. Could you also say your relationship beforehand and how old you were at the time? - thanks :)

Well I talked to her for a couple of days and asked her out. Then we broke up like 4 or 5 months later.
