So I've been depressed a long time now, so I decided to finally schedule a doctor's appointment for it and I want to know a few things.
What do they ask you when you go in for depression?
If I feel like I need medication do I have to ask for it?
Will I have to talk about my experiences with self-harm?
I know that you can't give exact answers, but just an idea would be nice.

I dont know how it all works in america, but this is my experience of it in england-
What do they ask you when you go in for depression? How you have been feeling, what you think triggered it, any problems you have (e.g lack of sleep or concentration)
If I feel like I need medication do I have to ask for it? I had to ask for mine, i would ask if you feel you need it otherwise they will choose to put you down the therapy route first (i have done both) so im not sure whether they will give you med straight away
Will I have to talk about my experiences with self-harm? If they ask you cant cover it up, i presume so, i know i did. But If you say you are uncomfortable talking about it then i dont think they'll press the issue
Hope this is helpful:)

If your going to see a psychologist, they are the ones asking you questions, and giving you advice and help you plan out your woes, troubles, etc teaching you methods to cope with what your feeling.
If your going to see a psychiatrist, they are the ones prescribing you medication for the symptoms of whatever you may be suffering from or feeling.
Hope that clarified some of your uncertainties.

Thanks guys I'm just nervous a bit. I know it's to help me get better, but I hate going to doctors in general.
