...some members have a plus symbol next to their name?

Yeah, or anywhere: visitors to your page, blog, etc. All friends :D

Well, sometimes I see my own name with two pluses, so maybe that is connected to this question.

(Oh, never mind. That's my user title.)

The two pluses next to member is a member title, the plus next to a name in the active box means they are on your friends list. :)
