You need to improve on your revision then. look up revision techniques or ask a teacher about it, im sure they would be more than happy to help and give you some ideas.
There isn't much else you can do to raise grades, just work as hard as you can and do one thing at a time, don't let things distract you.

How exactly do you study? As in what environment, what time etc.?

I find that when I rush work whilst watching tv or something I tend to do worse than when I sit down with some music on and work solidly. You need to discover what study technique works for you because clearly the one you have doesn't at the moment.

You also can see your teachers explain the problems you're having they can go through the topic area's you're struggling with and help you with extra practice.

Someone mentioned the TV. Unless you are watching a TV programme related to what your are studying I'd stay clear of the TV as it is a big distraction.

Apart from hard work you also need to make sure the work you put in is as productive as possible. There is little point in trying to fit study into five minutes here and there as it takes a little while to "get into the zone" where your brain is warmed up and thinking about the thing you are studying. On the other hand you can go for too long or try to study when you are tired from other things and this is also less productive. Find a time when you will have between 20 and 40 minutes uninterrupted and are felling reasonably fresh and awake.

We also learn by linking new knowledge to what we already know and understand. If you let yourself get behind in a subject when you first come across something you don't "get" subsequent learning is more difficult. As soon as there is something your don't understand ask extra questions of the teacher, someone else who does understand or research it yourself. Never believe for one moment that you will understand things when you come to revise them if you either did not understand them the first time round or didn't pay attention.

that's your problem then, you need to spend sufficient time studying or its pointless maybe drawing up a timetable will help put your school day in and any commitments you have then dependent on your workload and the tine you have find a few hours each day to work, turn your phone off while you do to stop yourself being distracted.
I know it seems boring or a lot of effort but if you wasn't good grades you're going to have to work for them.

This.

In addition to this, you should also ask your teachers about any extra credit assignments that you could complete but MyOblivion's suggestions should be done first, and then mine if you think you should.

Good luck!

You should try studying without the music and see if that helps you study any better. But I know some people who actually need distractions like that to help them study...if you're even more distracted without the music, obviously go back to it, as it may be helping you concentrate.

Maybe get a tutor. Also maybe try a different study technique? Like flash cards work really well for me

You should try different methods of study one example is studys have shown listening to classical music helps remember it. Also change up the environments.

Talk to your teacher(s). They usually will help you out however they can. Do you have severe test anxiety? They can help with that too. Teachers are always happy to see that a student cares enough about their grades to ask for help. They can then see that you're not just a slacker. Seeing your school counselor might help too. They might be able to identify study techniques that are better suited for you. People's brains are not all the same and therefor learn better with different things.

do all your homework!!! it may not help it go up, but it will stop your grade from falling down
