For me it depends on the risk of someone walking in. If the risk is low I will be completely naked with no covers. If it is more risky but I may get some prior warning then I'll have the covers over my legs but only half way up my thighs so my penis, stomach and chest are all uncovered but I could quickly pull the covers over.

I have done it under the covers, holding them up with one hand and stroking with the other but this is only when I would get no warning at all, like someone else sleeping the the same room.
