Yes i had once and i almost died.
My mom took me to the exam and when the doctor had started the exam someone knocked on the door. It was a friend of my mom who wanted to talk to mom and she had her daughter with her! So they like talked for 5-10 min when i had my exam.

Nope, while actually there was once 1 time where I was getting a physical and the curtain wasn't closed all the way and there were girls behind me, so one of them might have seen idk though.
