So I'm 134 lb. of skin, bones, and barely any muscle. I can only lift 25 lb. dumbbells and I'm not super strong, I've tried doing P90X 2x but I keep stop toward the middle. I have a disease which makes me have malnutrition so I'm kinda underweight the doctor says. I need to weigh about 14 lb. more than I already do.

I used to play Football with my friend, cause he was going to join the football team and he wanted me to help him train (when we were 13) and so when I got tackled I would get up and hear major ringing in my ears and usually twist my wrist or ankle.

What can I do to bulk up, while slim down? (what I mean is that I want to decrease body fat, while increase muscle)

I donT have any weights other than dumbbells.

What are the things you have to know in order to play football?

*ill be a junior by the time I play football (junior in high school)

How do I make my calves look muscular?

Talk to your coach and ask for help. That's what he's there for.
Push-ups, pull-ups, squats, jogging, sprinting, planks, walking. All free and all good exercises.
Bulk up while slimming down? More protein and healthy fats from meats, eggs, fish, nuts. Less bad/processed carbs from breads, pastas, grains, junk food.

Agreed with the proteins. Also, theres some powders and drinks you can get to drink along with your work out plan. Muscle Milk tastes the best, but it has the worst nutritional value for you.
