Quite simple.
If they break any of the rules, usually, you will get infraction points. These will show up in your user CP, I don't know where because I never had them. But anyway, the only way to get immediately banned is if you do a serious offense (porn, illegal drug promotion etc...) and that isn't very common. Any Admin has the right to permanently ban anyone without further reasoning, and Global Mods do this only if absolutely necessary. Also, there are users who can be frozen as well. This means that they are either below the age to enter VT (13+) or have committed enough small offenses for a temporary ban.

To add to what stopping time said if I remember correctly normal Moderators have the ability to ban but only for underage/overage but the highest a Mod can do otherwise is freeze someone which last for three days and is pretty much a ban until they are unfrozen or most usually banned.

I can ban, but only if overage.
The most common types of bannings we issue are excessive trolling, double-accounts, attempting to cam, and being a bot.
Basically, anything that is illegal in the USA or doesn't follow our rules can get you banned.

Quick question since I'm interested now, are both accounts banned when they make double-accounts or just the second one?

Second account is banned, first one is frozen.

Mods cannot ban. I don't know where you got that info from. They can do the next big thing which is freeze members. Global Mods can ban members. I'm not questioning your abilities, but from what I remember, regular mods cannot ban members.
You can get banned for a list of things. There's a forum rule thread around here somewhere. Find that. If you don't follow those, you will get banned.

Mods can give an infraction for overage members worth 100 points, which is an automatic ban

Yes they can, I banned multiple people when I was a moderator. Besides the overage ban, Mods can also give a person an infraction, which if when giving the infraction the reciever incurs more than 100 points, they will be banned. Thus, mods can ban people.

Yeah, mods can ban people. However when it comes to spammers or trolls, mods freeze and the accounts are checked by gmods/admins before being completely banned.

As Thomas said, the first will be frozen the second is banned. However there is differences. How long you're frozen for depends on how many accounts you have made.
1 account - 1 week frozen.
2 accounts - 2 weeks frozen
etc. Thats the way i've always done it anyway.
