I suppose logically the only issue is the possibility of concentrating really bad genes (i.e recessive ones) in any child that could result. Humans also have a sexual repulsion towards siblings. That repulsion is sort of diminished in adolescent years due to hormones and the such.

It's pretty messed up, but those fantasies are not rare.
