Okay everyone, recently I've been having an issue with my video card. Artifacts and crashing when using anything that's GPU accelerated. I've tried different drivers and OS re-install.

Tell us the basics:
What Graphics Card? Be specific. Preferably the whole model number; just the manufacturer won't help
What OS are you running? Please be specific. Example: Windows 7 Home Premium
Are you using a manufacturer driver or a community-contributed one(only applicable on Linux/BSD)
Have you searched Google(or your favourite search engine) for similar issues?
Please tell us all that, we will then be able to help you(hopefully). :)

Artifacts are never a good sign and always point to one outcome. A dead card. I would ring your manufacturer and ask for an RMA if its stil in warranty, if not its time for an upgrade :D

I've tried Windows 7 Ultimate x86 and x64

Yes, I successfully fixed it by under-clocking, but I'd like a more permanent solution.

Under-clocking, unfortunatly, seems like the best way to go about things. Perhaps your video card gets overloaded by the overclocking? You might have to replace it.
Have you tried Linux or BSD with the appropriate driver? Driver upgrade?

It may be the datacore or graphic card is getting old
